Dropsy
Definition: Dropsy is not a disease itself, but a serious symptom of internal organ failure, most commonly involving the kidneys. It occurs when a fish can no longer regulate fluids properly, causing dangerous internal fluid buildup. By the time dropsy is visible, the condition is usually advanced and potentially fatal without intervention.
Dropsy is most often caused by:
- Internal bacterial infections
- Chronic poor water quality
- Long-term stress or weakened immune systems
- Advanced parasitic or organ damage
It can affect both freshwater and saltwater fish, but it is far more common and visible in freshwater species due to osmotic imbalance.

In healthy fish, the kidneys regulate salt and water balance. When kidney function fails:
– Fluids leak into body cavities
– Internal pressure builds rapidly
– Organs become compressed
– Scales are physically forced outward
Once pineconing appears, internal damage is already significant.

Hospital tank with pristine water
Strong aeration and stable temperature
Internal antibacterial medication
Broad-spectrum antibiotics designed for internal infections
Follow the full course, even if symptoms improve
Epsom salt support
Helps draw excess fluid out of the body

Perfect water quality
Ammonia and nitrite at zero
Low nitrate
Frequent water changes if necessary

Quarantine immediately
Never treat in the main tank
Clean container with heater and air stone is sufficient
Epsom salt dosing
1 tablespoon per 5 gallons
Helps reduce swelling and internal pressure
Do not substitute aquarium salt
Daily water changes
25–50% daily with conditioned water
Stability matters more than exact volume
Temperature support
Slightly warmer (within species tolerance) to support immune response
Light feeding or fasting
Fast 24–48 hours
